Is delaying or combining prayers considered an act of disbelief (kufr), especially for someone with an excuse such as urinary incontinence ( سلس البول) which requires a long time in the bathroom?

The issue of someone who abandons prayer out of laziness does not render him an apostate according to the majority of scholars. However, combining two prayers without a valid excuse is considered a grave sin. If a person suffers from urinary incontinence, some scholars permit him to combine two prayers; otherwise, the obligation is to perform each prayer at its prescribed time.
