The answer

It is permissible to hold the Mushaf (copy of the Qur'an) to read from it during obligatory and supererogatory prayers. It is preferable to place it in front of the worshipper. If the Imam makes a mistake in reciting Surat Al-Fatihah that alters the meaning, the prayer behind him is invalidated. If the mistake is in something else and was not intentional, the prayer is valid. It is preferable for the one who recites the Book of Allah best to lead the prayer, based on the saying of the Prophet, peace and blessings be upon him: "The people are led in prayer by the one among them who is most skilled in reciting the Book of Allah."
