The question

What is the ruling on a patient with chronic vertigo breaking their fast in Ramadan in order to be able to pray? Is it permissible for them to pray with tayammum while in a state of janabah if they are unable to perform ghusl, or should they wait until they are able to perform ghusl? Is it a condition for them to wait until the vertigo attack subsides to pray with concentration, or should they pray while lying down to preserve the prayer time?

The answer

If fasting increases the likelihood of a husband experiencing dizziness, he is permitted to break his fast when he feels dizzy, or after experiencing it if that alleviates his condition. The illness that permits breaking the fast is one that causes evident hardship to the fasting person.

If he is afflicted with a dizzy spell while in a state of janabah (major ritual impurity), he must perform ghusl (ritual bath) for prayer. He is permitted to combine prayers by advancing or delaying them. If the spell continues and he fears the time for prayer will expire, he is permitted to perform tayammum (dry ablution).

A person is required to pray standing; if he is unable, then sitting; if he is unable, then lying on his side. It is not permissible to delay a prayer beyond its prescribed time except in the case of combining prayers for the sick.
