Is mentioning entering the washroom restricted to defecation, or does it include ablution and urination?

It is legislated for one entering the lavatory to say: "O Allah, I seek refuge in You from the male and female evil spirits (al-khubuth wal-khabaith)," based on the Hadith of Anas, which is agreed upon. This invocation is not exclusive to entering for defecation but includes entering to relieve oneself generally, even if the entry is for ablution (wudu').
