"Madhī" (pre-seminal fluid) nullifies ablution, and any part of the body or clothing it touches must be washed. There is no need to actively search for it unless one is certain of its emission.
As for "dafq" (gushing), it is close to the meaning of continuous flow. If the fluid does not exit with a gush, then it is not semen but rather madhī.
It is sufficient to immerse the limb in water during "istinjā'" (washing after relieving oneself), and there is no need for excessive effort beyond that.
If purifying madhī in winter causes harm, then the obligation of sprinkling water is waived. However, it is not thought that the matter would reach the extent of causing harm.
