Missed and Made Up Prayers

• Is prayer obligatory to be repeated if some of its pillars - such as devotion - were missed due to the imam's speed?
• How does one make up two combined prayers for an excuse after their invalidity becomes apparent: Is it obligatory to combine them with the intention of combining when making them up, or is it permissible to make up each prayer separately without the intention of combining?
• How can one atone for the sins of masturbation and abandoning prayer due to ritual impurity (janabah), and what is required after breaking an oath to quit masturbation?
