What is the wording for Du'a al-Qunut, and in which prayers is it recited?

Formulas for the Qunut supplication and its positions according to the Four Imams:

1. Al-Shafi'i: The formula for the supplication is: "O Allah, guide me among those whom You have guided... to its end."It is recited in the Fajr prayer (in the plural form for the Imam), in the Witr prayer, and during calamities in all prayers. 2. Abu Hanifa: The formula for the supplication is: "O Allah, we seek Your help and Your guidance... to its end." It is recited only in the Witr prayer, and during calamities and misfortunes in the five daily prayers. It is recited before the bowing (ruku') of the last rak'ah. 3. Malik: He agreed with Abu Hanifa that Qunut is before the bowing (ruku'), but in the Fajr prayer and silently. 4. Ahmad: He does not perform Qunut except in the Witr prayer, and after the bowing (ruku'), raising his hands, and also during calamities.
