Is it permissible to combine two prayers if there are no clean places to pray and the water is very cold in winter?

The earth is pure, and it is permissible to pray on it, except for graveyards and bathrooms. If it is assumed that there is no pure place, one can pray on a pure prayer rug. If that is not possible, it is permissible to combine two prayers according to the Hanbalis, but the majority of scholars do not permit it. As for bathrooms, their purity has no bearing on the validity of prayer.
