If her refusal is due to illness or harm, then there is no sin upon her. Intercourse with a pregnant wife is permissible as long as there is no harm to her or the fetus. This should be determined by a doctor's report. The husband should consider his wife's health and psychological state during pregnancy. A wife's refusal when her husband has a strong need is forbidden. The general principle is that intercourse does not harm the fetus, and excessive fear is not to be heeded. The matter should be handled with wisdom. The husband should be considerate of his wife, and the wife must obey her husband unless harm is feared, which should be determined by a specialized doctor. It has been reported from the Prophet, peace and blessings be upon him, a stern warning regarding a wife's refusal to her husband, stating that angels curse her until morning if her husband spends the night angry with her. There is no objection to the husband being present at his wife's birth, as it is permissible for him to look at her body. However, he should not do so if it requires him to look at the 'awrah (private parts) of the female doctor or anything else that is impermissible for him, and there is no necessity for his presence.
